CWMTF Master File Documentation Update Form

Clean Water Management Trust Fund (CWMTF) maintains a central file of contract documentation for your organization in an effort toward efficient operations and paper reduction. It is the responsibility of your organization to send our office any updates, amendments, and/or changes to these documents.
Please note below the status of these documents over the past twelve (12) months. Check the appropriate corresponding boxes, sign the form, and attach the required documents as necessary. 
**New for 2020** If your organization is using electronic funds transfer (EFT), please verify that the correct contact and banking information is being used. To do so, complete the Vendor Electronic Payment form and fax or email it to the Office of the State Controller.
