By becoming a member of AFA Basketball with Adams Fundamentals Academy LLC, you are agreeing to the following terms/conditions below.

*If you do not agree with our terms & conditions - we ask that you do NOT enroll into our program. Our terms are extremely clear and protect the integrity of our program. Our program is selective and certainly NOT for every family. We only want to work with families who truly buy into our culture at Adams Fundamentals Academy LLC*

Please follow the steps below to move forward.

Step 1: Please read our terms & conditions entirely to understand the expectations of AFA's Basketball Program. If you have any questions or concerns, email Coach Mario at AFACoaching14@gmail.com to schedule a phone call.

Step 2: Once you have reviewed this document, you simply agree to all by filling out the required fields below. Type your First and Last name into the boxes, along with your email and phone number, then input your athletes name. Next, checkmark the box that you agree to our policies in this document and finalize with your signature. Our terms & conditions are showed and agreed to BEFORE making payment. 

Step 3: Return to the welcome email you recieved and continue the next step by click the payment link to finish the process. 

 AFA Basketball Terms and Conditions 

This agreement will begin on the day you register and continue for either 3, 6 or 12-month agreements. Depending on your selection of duration, all funds will be electronically billed on a recurring basis for any upfront payments,monthly payments, or free trial services through our subscription membership.

If you want to cancel your agreement during your current contract, then the termination fee will be the remainder of your contract agreement, once that amount is paid then your card will be removed from our systems. If you choose to cancel after 3, 6 or 12 months, I agree to provide Coach Mario with a 30-day typed notice at Afacoaching14@gmail.com to cancel any future recurring payment after the contract is complete. Once again this is for all upfront, monthly, or free trial subscriptions. If you do not follow our terms, you will be automatically enrolled into the same agreement for the next 3, 6 or 12 months and your card will be billed for the amount you originally purchased. We will not offer any refunds or reimbursements. (No exceptions) Coach Mario will send you the 30 day cancelltion date, start and end dates after you have registered your athlete for our program. It is up to you as the parent to keep track of those important dates for your reference.

By registering for any AFA programs, you agree to the terms and conditions, player expectations and the parent expectations that clearly state Coach Mario’s refund, cancellation, and early termination policies. 

Please read the below player expectations to your athlete so they are crystal clear of the rules.

AFA Basketball Player Expectations 

• I agree to be a positive player who is coachable
• I agree to bring my journal to EVERY session to document my progress
• I agree to work hard in every session, I’m here to develop and reach my goals as a player
• I agree to complete each homework tasks that Coach Mario assigns me in a timely manner
• I agree to be accountable to Coach Mario’s program and not make excuses
• I agree to bring something to drink and my own basketball (and proper attire) to each session
• I agree to work hard and achieve my personal goals that Coach Mario and I set on day 1
• I agree to be committed every week and dedicate time to work on my own away from our sessions
• I agree to use the online players community to stay updated and accountable for my progress
• I agree that I can communicate with Coach Mario daily about my progress inside of the members area that I have unlimited access to and ask questions under the “Player/Coach questions” tab
• If I am enrolled in the group training, I agree to schedule a 1 on 1 monthly check up in the players community tab with Coach Mario to discuss my progress and development
• I agree to put in the work needed to become a better player. My results are my responsibility.
• I agree to fill out my weekly “game evaluations” so Coach Mario and I have a deep understanding of my performances throughout the season
• I agree to be a positive player when I make mistakes at the sessions, I agree to not become a distraction or disrupt other players with poor behavior. If my behavior is disrepsectful, harmful, or erratic in any way towards other players/ coaching staff, I will immediately be removed from all AFA training programs.
• I agree to arrive 15 mins early to every training session to stretch and get dressed

AFA Basketball Contract Policies

• Communication Policy: Once we become a member, all communication is done through the Skool app (that you receive access to once you enroll your child). No text messages or phone calls will be answered. If you have any personal questions that you do not wish other parents to view, please email us at AFACoaching14@gmail.com or set up a time to talk via zoom.
• 30 Day Cancellation Policy: All AFA memberships are set up as recurring subscriptions/payments. This applies to both monthly, upfront, or free trial payments. To cancel your contract/payment renewal, you must give a 30-day written notice prior to your contract renewal date in order to cancel your contract and subscription payment with Adams Fundamentals Academy LLC. Failure to do so will result in automatic renewal of a 3-6- or 12-month commitment of your contract and your card being charged for whichever option you originally chose (up-front or monthly) and will not be refundable or reimbursed, no exceptions. Cancellation cannot be done in person; notice must be in written communication (via email) to AFACoaching14@gmail.com 30 days or more prior to your contract renewal day/next automatic renewal payment date.

- For Example: If your contract/next payment is set to auto-renew on Dec 1st. You must cancel on November 1st or sooner in writing via email. November 2^nd would be considered late & the auto renew payment on Dec 1st will go through and your card will be charged.

• Payment Policy: Payment will be taken using our Sam Cart checkout page which works with stripe to process your credit or debit card. Depending on the option you choose, your card will be charged every 3 months, 6 months, or 12 months. Our current payment options are in the AFA Pricing document for you to review. Payment is automatically renewed, and your card will be charged unless canceled in writing 30 days prior to your next payment date. Once again, you must send an email to AFACoaching14@gmail.com to turn off the recurring billing which will cancel your contract at the end of the agreement.
• Dates/Time Reminders Policy: It is the responsibility of the client (Parent) whose player is a member of any training programs associated with AFA Basketball, to set time/date reminders that pertain to your subscription. Coach Mario will email you those dates after you have registered for the program. If you'd like to know a specific date you may reach out at anytime via email at AFACoaching14@gmail.com

• No refund/credit policy: We do not provide refunds/credits for training sessions missed as a result of injury, illness, scheduling conflict, emergency or any other event out of Coach Mario and AFA Basketball's control. All sales are final. Your athlete's spot is also not transferable to anyone else (including siblings). Any session not attended within this specific training program (unless you follow our makeup session form rules) is forfeited. There is always a risk of unexpected events coming up that may prevent your athlete from attending/participating. By becoming a member of our program, you understand this & take on the risk. If your child's behavior is disrepsectful, harmful, or erratic in any way towards other players/ coaching staff, he or she will immediately be removed from all AFA training programs and there will not be a refund for any subscriptions that have not been used.
• Failed Card Payment Policy: If your card payment fails you have 12 hours to update it. You will automatically be sent an email to update the card on file. All training by your athlete will stop until the payment has gone through.
• Rate Policy: The investment of the program is always subject to change for anyone that is not enrolled in our program or who cancels their subscription/membership.
• No Freeze/Pause Payment Policy: We do not freeze, pause, or extend subscriptions/payments.
• Results: We do not guarantee any results. Results will vary based on each player's effort, energy & time they put into the program both in person as well as using our online homework academy. Results are up to each player.
• Program Changes Policy: We reserve the right to make changes to the program at any time.
• Time Policy: Time is important. Please bring your athlete 15 minutes prior to our start time so they are able to warm up & shoot around. We always start & end exactly on time. Any time missed is considered forfeited. Our sessions are 60 minutes in duration.   
• Player Expectations: Each player is expected to be coachable, positive, work hard & train on their own using our Skool community by completing their homework assignments and using the resources available to them. Junior Group Players are expected to follow instructions, listen, and work hard.  Each player's results are up to them & is their responsibility. Bring your journal, pen, basketball & water to each session.

AFA Basketball Parent Expectations

• If you (as a parent) have questions during the week, you can ask questions inside of the Skool app 24/7 and will receive a response within 12 hours (Monday-Friday) or via email at AFACoaching14@gmail.com
• If you need to have a 1-on-1 call - you can schedule a 10 minute zoom call with Coach Mario through the Skool app under the “Parents schedule a call with Coach Adams” in the parents community. All calls will be recorded.

• I agree with Coach Mario’s reschedule policy, meaning if you miss that session - we don’t offer a private 1-1 or group makeup session. However, if you submit the “Makeup Session Form” 48 hours before our session stating your child can’t make it to our assigned session - we WILL allow you to come train on the last Sunday of the month from 2:30-3:30 pm. If you do not submit the form within our members area - you will forfeit the session.
• I understand that my child has 24/7 access to Coach Mario inside the Skool Community area to ask him any questions and receive daily mentorship throughout the week. 

• I understand that if I decide to attend my child’s sessions, I will not pressure my child or yell from the sidelines. We train in a “non-pressured” environment. If such an event happens, we will issue a warning to the said parent, a second offense will be the removal of the parent from our training sessions.
• I understand that Coach Mario encourages all the kids to attend Sunday Zoom meetings at 5pm. If your son or daughter cannot make it, they will be recorded in the players community. 
• I understand how to communicate with Coach Mario (and set up a 10-minute scheduled call when there are conflicts/vacations in advance) via zoom inside the parent’s community so we can better prepare for our sessions.
• I understand that if we are “out of town” or “on holidays, AAU tournaments, that we are still a member of the program. Sessions do not pause and will continue based on the commitment you have agreed to.  Please communicate with Coach Mario ahead of time when you are going out of town so we can plan accordingly by providing homework and recorded lessons.
• Currently our program offers 1 on 1, and Semi Private training yearly, Monday-Thursday, and Group sessions are on Sundays all year. If you can’t make it to a session that week or weekend - we do allow players to come to the last Sunday of the month (unless it’s a major holiday or otherwise communicated) and train for 1 hour if and only if you (as a parent) complete the Makeup Session Form inside of our members area stating, you can’t make it that week 48 hours BEFORE the session. The makeup session is in a group format with other players in our academy. 
• I understand that 1 on 1 clients and Semi Private clients will have the last week of each month off, unless stated on the calendar inside the Skool Community.

• I understand that Coach Mario takes a vacation for the full month of July every year, at which point AFA Assistant Coaches will take control of training sessions until Coach Mario Returns August 1st.If this is an issue, please notify us immediately. This allows Coach Adams time to re-plan programs for the next year.
• Dates that Coach Mario will not have 1 on 1 training and group training are located inside the Skool community Calendar and the Holiday Days Off Tab in the classroom. 
• I understand that we will complete either 12, 24 OR 48-52 sessions within the 3, 6- 12-month agreement.
• Missed sessions do NOT roll over into the following year/term for any reason. 
• I understand that Coach Mario’s billing process is an automatic electronic funds transfer through SAMCART and Stripe.
• I understand that if I have a “failed credit/debit card payment” I will need to register with a new credit or debit card within 12 hours of the failed payment. Sessions with your child will pause until the payment is collected.
• We offer two options for payment. 1) Pay in full or 2) Monthly Subscription. All sales are final. If you enroll and don’t use the sessions - that is at your discretion. All AFA Programs are subscriptions (whether you pay in full/upfront or monthly, your payment will be set to recurring billing within our billing system) Once again we do not offer refunds, reimbursements for any transaction during this process.

• I hereby grant permission to Adams Fundamentals Academy to photograph me or otherwise have images or voice recordings made (including but not limited to digital photographs, video or digital moving images and/or voice recordings), for AFA publication or promotional purposes in any medium (including but not limited to print media, television, video, motion picture, or Web site on the Internet). I additionally consent to the use of my name and/or interview comments in connection with Adams Fundamentals Academy publications or promotional purposes in any medium.
• WAIVER AND RELEASE OF LIABILITY
  In consideration of the risk of injury while participating in basketball training (the "Activity"), and as consideration for the right to participate in the Activity, I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily enter into this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in the Activity, and do hereby release and forever discharge Adams Fundamentals Academy, located at 326 Julie Dr, Moscow, Idaho 83843, their affiliates, managers, members, agents attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any physical or psychological injury, including but not limited to illness, paralysis, death, damages, economical or emotional loss, that I may suffer as a
  direct result of my participation in the aforementioned Activity, including traveling to and from an event related to this Activity. I AM VOLUNTARILY PARTICIPATING IN THE AFOREMENTIONED ACTIVITY AND I AM PARTICIPATING IN THE ACTIVITY ENTIRELY AT MY OWN RISK. I AM AWARE OF THE RISKS ASSOCIATED WITH TRAVELING TO AND FROM AS WELL AS PARTICIPATING IN THIS ACTIVITY, WHICH MAY INCLUDE, BUT ARE NOT LIMITED TO, PHYSICAL OR PSYCHOLOGICAL INJURY, PAIN, SUFFERING, ILLNESS, DISFIGUREMENT, TEMPORARY OR PERMANENT DISABILITY (INCLUDING PARALYSIS), ECONOMIC OR EMOTIONAL LOSS, AND DEATH. I UNDERSTAND THAT
  THESE INJURIES OR OUTCOMES MAY ARISE FROM MY OWN OR OTHERS' NEGLIGENCE, CONDITIONS RELATED TO TRAVEL, OR THE CONDITION OF THE ACTIVITY LOCATION(S). NONETHELESS, I ASSUME ALL RELATED RISKS, BOTH KNOWN OR UNKNOWN TO ME, OF MY PARTICIPATION IN THIS ACTIVITY, INCLUDING TRAVEL TO, FROM AND DURING THIS ACTIVITY.
  I agree to indemnify and hold harmless Adams Fundamentals Academy against any and all claims, suits or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf, including attorney's fees and any related costs, if litigation arises pursuant to any claims made by me or by anyone else acting on my behalf. If Adams Fundamentals Academy incurs any of these types of expenses, I agree to reimburse Adams Fundamentals Academy. I acknowledge that Adam Fundamentals Academy and their directors, officers, volunteers, representatives and agents are not responsible for errors, omissions, acts or failures to act of any party or entity conducting a specific event or activity on
  behalf of Adams Fundamentals Academy. I ACKNOWLEDGE THAT THIS ACTIVITY MAY INVOLVE A TEST OF A PERSON'S PHYSICAL AND MENTAL LIMITS AND MAY CARRY WITH IT THE POTENTIAL FOR DEATH, SERIOUS INJURY, AND PROPERTY LOSS. The risks may include, but are not limited to, those caused by terrain, facilities, temperature, weather, lack of hydration, condition of participants, equipment, vehicular traffic and actions of others, including but not limited to, participants, volunteers, spectators. coaches, event officials and event monitors, and/or producers of the event.
  I ACKNOWLEDGE THAT I HAVE CAREFULLY READ THIS "WAIVER AND RELEASE" AND FULLY UNDERSTAND THAT IT IS A RELEASE OF LIABILITY. I EXPRESSLY AGREE TO RELEASE AND DISCHARGE Adams Fundamentals Academy AND ALL OF ITS AFFILIATES, MANAGERS, MEMBERS, AGENTS, ATTORNEYS, STAFF, VOLUNTEERS HEIRS, REPRESENTATIVES, PREDECESSORS, SUCCESSORS AND ASSIGNS, FROM ANY AND ALL CLAIMS OR
  CAUSES OF ACTION AND I AGREE TO VOLUNTARILY GIVE UP OR WAIVE ANY RIGHT THAT I OTHERWISE HAVE TO BRING A LEGAL ACTION AGAINST Adams Fundamentals Academy FOR PERSONAL INJURY OR PROPERTY DAMAGE.
  To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence on the part of Adams Fundamentals Academy, its agents, and employees.
  In the event that I should require medical care or treatment, I agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and understand that I should carry my own health insurance. In the event that any damage to equipment or facilities occurs as a result of my or my family's willful actions, neglect or
  recklessness, I acknowledge and agree to be held liable for any and all costs associated with any actions of neglect or recklessness. This Agreement was entered into at arm's-length, without duress or coercion, and is to be interpreted as an agreement between two parties of equal bargaining strength. Both the Participant, and Adams Fundamentals Academy agree that this Agreement is clear and unambiguous as to its terms, and that no other evidence will
  be used or admitted to alter or explain the terms of this Agreement, but that it will be interpreted based on the language in accordance with the purposes for which it is entered into.
  In the event that any provision contained within this Release of Liability shall be deemed to be severable or invalid, or if any term, condition, phrase or portion of this agreement shall be determined to be unlawful or otherwise unenforceable, the
  remainder of this agreement shall remain in full force and effect, so long as the clause severed does not affect the intent of the parties. If a court should find that any provision of this agreement to be invalid or unenforceable, but that by limiting said
  provision it would become valid and enforceable, then said provision shall be deemed to be written, construed and enforced
• I understand (according to Coach Mario’s liability release form) Adams Fundamentals Academy and any assistant coach for his training program is not liable for any injuries.
