PCS Student Withdrawal Form

We will miss having your child(ren) with us, but thank you for taking a few minutes to fill in this form.  Your comments will be used to help us continue moving forward with "Educational Excellence to the Glory of God".
Note that parent(s)/guardian(s) are required to give a minimum of one school month's notice of a student leaving or the following month's tuition will be charged.  This means that families who notify us after June 1 that their child(ren) will not be returning to PCS in September will be responsible for September fees.  This does not include students who are graduating from grade 12.
