With the submission of required immigration documents below and upon official approval of your reinstatement application a new Form I-20 will be created.
Notification of official approval will be provided by the Registrar's Office.
Deadline to submit immigration materials to ISSA:

If all documents are complete and accurate your I-20 will be created within three weeks of official approval notification.
Please complete and submit the following documents to move forward with processing your new Form I-20. You will be contacted should any documents need to be updated.
1) Application for Form I-20 (Below)
2) Declaration of Finance (Linked Below after you indicate your Degree Level)
3) Biographical Page of Passport
4) International Student Responsibilities Agreement (Below)
After these materials have been received and if your reinstatement application is approved, a new I-20 will be created and emailed to you. If your F-1 Student Visa has expired, you will then take your I-20 with your passport, financial documentation and your SEVIS fee Payment Receipt to the U.S. Embassy or Consulate in your home country to apply for a new F-1 Student Visa.
Once you arrive on campus, it is mandatory that you complete the Arrival Confirmation Form so that we can register you in the Student and Exchange Visitor Information System (SEVIS). We will send a follow up email with the link for this!
