Sports & Arts, LLC Recreation Center Activity Waiver and Release of Liability

Acknowledgment and Assumption of Risk By signing this waiver, I, the undersigned parent or legal guardian, acknowledge and understand that participation in activities at Sports & Arts involves certain risks, including but not limited to injuries arising from:

• Physical activities such as soccer, dance, and recreational games.

• Use of equipment, including but not limited to balls, paint supplies, and game tables.

• Interaction with other participants.

I voluntarily agree to allow my child to participate in activities at the Sports & Arts Center and assume all risks associated with these activities, whether known or unknown.

Medical Treatment and Emergency Care I give permission for the staff of the Sports & Arts to administer basic first aid in case of an injury or medical emergency involving my child. If further medical care is required, I authorize staff to contact emergency medical services. I understand that I am responsible for any medical expenses incurred as a result of my child's participation.

Release of Liability In consideration of my child’s participation in activities at Sports & Arts, I agree to release, waive, discharge, and hold harmless Sports & Arts, its owners, employees, volunteers, and agents from any and all liability, claims, demands, or causes of action, including negligence, that may arise from participation in Sports & Arts activities.

Media Release I grant permission for Sports & Arts to take photographs and videos of my child while participating in activities at the Center. These images may be used for promotional purposes, including but not limited to the Center's website, social media platforms, and printed materials. I understand that I will not receive any compensation for the use of these images.

Code of Conduct I acknowledge that all participants are expected to follow the rules and guidelines established by the Center to ensure a safe and positive experience for everyone. Failure to adhere to these rules may result in dismissal from the program without a refund.

Parent/Guardian Consent I have read and understand the terms of this waiver. By signing below, I agree to the terms outlined above and confirm that I am the parent or legal guardian of the participant. I further affirm that I have explained these terms to my child as appropriate.
