Please carefully read the following information before submitting your registration form. 

Campers who are between the ages of two and twelve are eligible to attend Summer Camp; however, campers must have turned two by May 31, 2022.

Summer Camp will be offered in weekly sessions.  We offer both a half day summer camp (9am-1pm at $200) and a full day summer camp (9am-4pm at $250). Each week we offer a different themed camp. Though the theme is the same across all age groups, the activities will be adjusted to work for their age.

Session 1: May 31-June 3, 2022 (There is NO camp on May 30, 2022)

Session 2: June 6-June 10, 2022 

Session 3: June 13-June 17, 2022

Session 4: June 21-June 24, 2022 (There is NO camp on June 20, 2022)

Session 5: June 27-July 1, 2022

Session 6: July 5-July 8, 2022 (There is NO camp on July 4, 2022)

Session 7: July 11-July 15, 2022

Session 8: July 18-July 22, 2022

Session 9: July 25-July 29, 2022

Session 10: August 1-August 5, 2022

Campers attending summer camp are asked to bring their lunches each day. Snacks will be provided by the Weekday School. We are a NUT FREE school. 

There is a one time activity fee of $100 which is non-refundable. This fee helps cover your electronic payment fees, daily provided snacks, daily supplies and office administrative fees. This fee is due at the time of registration. The cost of each week is the same regardless of the theme. A 50% deposit is due at the time of registration. The remaining cost are due by May 20, 2022. 

 

Half Day Camp is 9am-1pm with a cost of $200 per week.

Full Day Camp is 9am-4pm with a cost of $250 per week.

At this time we do not accept credit card payments. Payments are electronically deducted from your bank account. A completed ACH form with a voided check is due within 72 business hours of registration.

Refunds/Cancellations:

All refunds/cancellations must be requested by May 20th, 2022. All refunds are subject to a $25 service fee. Requests received after this date will not receive a refund unless accompanied by a doctor’s note stating that the child will not be able to attend. If medical reasoning is diagnosed as COVID, then proof of a positive PCR test is required with cancellation.

In order to register for Summer Camp, this registration form must be submitted along with the activity fee ($100) and a 50% deposit.  Additionally, an authorization form, health forms and proof of identity and age (orginial birth certificate or passport) are also required to complete registration for Summer Camp.  The Authorization Form and the Commonwealth of Virginia School Entrance Health Forms can be found on our website under the enrollment tab:  www.stmatthewsumc.org/wds.

Current WDS families DO NOT need to complete an Authorization Form unless information has changed and, DO NOT need to  provide proof of identity and age as they are already on file.  Returning students must have a current physical and up to date immunizations on file. 

Children new to Weekday School programs must submit all forms in order to complete Summer Camp Registration:  Registration Form, Authorization Form, Physical and Immunizations and show original proof of identity and age. Please schedule a time prior to Summer Camp to submit all required documentation.

Emergency Information

Registration, Payments and Refund Policy

To register your child, please complete this form and submit it along with a payment authorization form, a voided check and the non-refundable activity fee of $100.  
 
You must also provide a 50% deposit for each week to hold your child's spot. This payment will be done electrionically as an auto withdraw from your bank account using the payment authorization form/voided check. 
 

Refunds/Cancellations:

Health Forms

Commonwealth of Virginia School Entrance Forms for all students are required by the Fairfax County Health Department on an annual basis. These forms must be dated in 2021 or 2022 and be on file by the first day of the session.  All children must be up to date on immunizations.  Additionally, an Authorization Form is required.  The Authorization Form can be found under the enrollment tab on our website:  www.stmatthewsumc.org/preschool-wds

Proof of Identity and Age

Proof of identity and age must be provided for each child enrolled in a Weekday School program including Summer Camp.  Proof of a child's identity and age may include a certified copy of the child's birth certificate, notification of birth (hospital, physician or midwife record), birth registration card, passport, copy of the placement agreement or other prooof of the child's identify from a child placing agency, copy of the entrustment agreement conferring temporary legal custody of a child to an independent foster parent, school record from a public school in Virginia, or certification by a principal or his/her designee of a public school in the U.S. that a certified copy of the child's birth record was previously presented.  If documentation has previously been provided to the Weekday School, then new documentation is not necessary.

I understand that my $100 activity fee is non-refundable once registered.
I have read and understand the "Registration and Payments and Refunds" section on this form (above).
Consent for Photography I give permission for photographs of the above named child to be taken during St. Matthew's Weekday School events.  These images could be used in publications, multimedia presentations, included on our web page, St. Matthew's Weekday School Facebook page, on Shutterfly Class sharesites, or stored for the purposes of archiving.  Web images will not identify child by name and will not be used for commercial purposes.
Whenever my child becomes ill, I will make arrangements to have my child picked up immediately from summer camp. I will review the St. Matthew's Weekday Schools COVID and illness policy as well. 
Consent for Medical Treatment: In the event that the child named above is injured and requires the attention of a doctor, I consent to the rendering of routine or emergency medical/dental care necessary to preserve the health of my child, including diagnostic, medical and surgical treatment by authorized members of a outpatient, emergency or hospital staff or their designees, as may be in their professional judgment to be necessary.  In the event treatment is required from a physician and/or hospital personnel designated by the Church, I agree to hold such person free and harmless of any claims, demands, or suits for damages arising from the giving of such consent.  I also acknowledge that I will be ultimately responsible for the cost of any medical care should the cost of that medical care not be reimbursed by my health insurance provider.  I also authorize first aid to be administered as judged to be needed by St. Matthew's staff.
