This site will allow you to send our medical team valid proof of your vaccination, negative COVID PCR or Antigen test or proof of previous exposure, before your arrival to camp. Use this site if you are attending one of our upcoming camps as a camper, counselor, church staff staying in Hume lodging or a volunteer.
For All Locations
Please Read Before Proceeding
Vaccination verification must show that the final dose was administered at least 14 days prior to arrival.
PCR Testing submissions must be dated by a testing center NO EARLIER than 72 hours (3 days) prior to camp.
Rapid Antigen Testing submissions must be dated by a testing center NO EARLIER than 72 hours (3 days) prior to camp, however we strongly recommend testing within 24 hours (1 day) before camp.
Tests administered earlier than 3 days immediately prior to camp will be considered invalid.
All Hume Lake locations will not accept self-reported, self-verified test results, test results utilizing mailed-in samples, or results from 'over the counter' purchased tests.
Those who have had COVID-19 in the past 90 days are exempt from testing requirements if able to submit lab results and more than 10 days have passed since a positive result.
For California Locations
Hume Lake and Hume SoCal do not accept antibody tests as a valid screening for camp.
Hume Lake and Hume SoCal do accept both negative PCR and Antigen tests for valid for screening. PCR is the best option if possible.
For Massachusetts Location
Hume New England does not accept Antigen tests, only PCR tests will be considered valid for screening.
If you are providing documentation for multiple individuals, please use a separate form submission for each individual.
