Please complete the following form and provide documentation of the cost-effective measures that were implemented. Owners must implement all cost-effective measures (any measure with a payback period of two years or less with rebates) identified through retro-commissioning within two years of the study. Submissions will be reviewed by city staff and confirmation of compliance with the Measure Implementation requirement will be sent following review.

Acceptable forms of documentaion include, but aren't limited to: invoices, receipts, photos, commissioning reports, M&V reports, narrative memo signed by a licensed professional, screenshots of controls, etc.
