AIRFARE REQUEST FORM

Use this form to request a live TravelPerks agent to assist you with making your airfare reservation. There is a non-refundable fee of $120 for each domestic reservation (not each person) and $150 for international reservations.

This form allows you to provide us with your exact passenger information in an easy-to-follow format. Use of this form is mandatory. You must enter your information exactly as it appears on your travel documents.

Note: Submitting your information does not imply that you have a reservation or that we will be able to successfully make a reservation for you. The fee is for our work trying to assist you, regardless of whether that work results in a reservation.
