I hereby grant to Hanger, Inc., and its direct and indirect subsidiaries, affiliates, employees and agents (collectively, "Hanger"), the absolute, perpetual and irrevocable right and permission, with respect to (i) any images, depictions, photographs, or video or audio recordings, of me or in which I may be included with others, including, without limitation, in any media or form of communication, captured or taken by Hanger or given by me or on my behalf to Hanger, including, without limitation, television, radio, satellite radio, newspaper, magazine, periodical, website, internet and computer media (e.g., hard drives, removable drives, zip disks, thumb drives, CD-ROM or CD-R discs, DVDs, flash memory, USB drives and floppy disks) (collectively, "Media"), (ii) any of my written or oral statements given by me or on my behalf to Hanger, regardless of the form of Media ("Statements"), and (iii) my name and biographical information given by me or on my behalf to Hanger ("Personal Information," together with any Media and Statements, are collectively referred to hereafter as the "Materials"), to use, re-use, broadcast, control, reproduce, print, publish (including, but not limited to, social media and/or video sharing platforms), exhibit, alter, copy, license such Materials in whole or in part, individually or in conjunction with other images, audio or written material (whether in written, oral, electronic or other form), in any media and for any purpose whatsoever including, but not limited to, Hanger's internal, educational, promotional and marketing activities. I understand that the Materials may contain my name, image, likeness, biography, statements, signature, words, voice, movements and/or other identifying characteristics or data.

I hereby agree that any Materials (including, without limitation, the storage thereof) shall constitute the sole property of Hanger with full right of disposition in any manner whatsoever, and I hereby irrevocably transfer and assign to Hanger any right, title and/or interest that I might have in or to any such Materials. I represent that the contents of all Statements made by me and any Personal Information that I have provided are true and correct to the best of my knowledge and belief.

I understand and agree that Hanger has no obligation actually to use the Materials prepared under the terms of this authorization and release, and has no obligation to use my name or any other form of attribution in connection with any Materials that may be used. I understand I will not receive any payment or other consideration in exchange for signing this authorization and release or for the use or disclosure of any of the above Materials.

I realize, of course, that in producing educational, promotional, marketing and other materials and/or presentations of any nature, Hanger does not intend to defame me. Nevertheless, to encourage Hanger to produce such materials and/or presentations in any manner that it may deem best, I hereby agree that I will not make or assert against Hanger or its assigns or licensees any claim or demand whatsoever based on the ground that anything contained in those materials and/or presentations defames me. I indemnify, hold harmless and forever release and discharge Hanger from any and all claims, liabilities, damages or expenses (including attorneys fees' and disbursements) arising out of, in connection or otherwise relating to the use of the Materials, whether for defamation, violations of privacy rights, infliction of emotional distress or anything else.

This authorization and release shall inure to the benefit of Hanger and to its licensees and assigns. I am over the age of twenty-one (21) or have received consent from my parent or guardian as evidenced by his or her signature below. I have read the foregoing and fully understand the contents hereof. I sign this authorization and release freely, knowingly and voluntarily, without any legal reservation, duress, coercion or undue influence, and fully intending to be legally bound thereby.
