* WAIVER AND RELEASE In consideration of and exchange for the City of Lebanon, Tennessee (the "City"), allowing the Wilson United Soccer League (WUSL formally LYSA) use of their property, for the WUSL soccer program, and for WUSL soccer program, and for WUSL allowing the Player and/or Player named to participate in such program, Player and/or Player's parent(s), custodial parent or guardian, does hereby, permanently and completely, forever waive and release any and all claims and causes of action against the City and WUSL, its Board of Directors, collectively or individually, for personal injuries to Players, or to property of Player, which player might have or hereafter acquire, as a result of Player's participation in the WUSL soccer program. Players and Player’s parent(s) or guardian, fully acknowledge that involvement in sports activity may be physically hazardous and that Players and Player’s parent(s) or guardian voluntarily assume the risk of any injury to player as a result of Player’s participation in WUSL soccer program. Player will not hold the City and/or WUSL, its board members, collectively or individually, responsible for the condition of property utilized for the soccer program, the condition of equipment, the operation of the program, and the conduct of Players, coaches, officials, spectators or any other cause which might give rise to injury or damage to Player. If Player is a minor or under a legal disability, this Waiver and Release is executed by Player’s parent, custodial parent, or legal guardian pursuant to all terms and conditions recited herein. The acceptance of one parent, custodial parent or legal guardian below shall be inclusive of any other parent, custodial parent or legal guardian of the minor.
