Donation Mini-Grant Program Application Form

The 'save-and-return' web-form requires a username and password which will apply to all current and future DMG applications.
If you have submitted DMG applications on this platform in the past, log in using that username.
For new users, we recommend using the Gmail or Microsoft account option, or your email as username.
Once you create a profile for the DMG program, you will not need to make a new one in future cycles.
Once the username and password is created, you can save and return to your application(s) in progress.
DMG applications must be submitted by midnight on the deadline: January 15, April 15, and August 15 each year.
